We have revised our Application for Approval of a Program for CEU accreditation.

To submit electronically:

 

1.  Type in all the information requested.  For Y/N questions, delete the Y or N, leaving only the one that is the proper answer. 

 

2.  For programs with multiple dates/locations, please submit a separate document (in excel or Word format) that lists the information in the following manner, so that it may be posted on our website:

 3.  Do not worry about formatting issues that may arise when the information you need to enter is larger than the space provided.  The blocks will expand down to accommodate the session information in the tables.  If this makes the tables more than one page that is fine.

State how clinical validation studies impacted algorithm and feature development and illustrate

a commitment to proven technology.

 

 

For session speakers you may choose to use the method shown in (1) and attach a separate electronic file of all the profiles for program speakers/presenters

 

For educations objectives and presentation format, please put the information in the table.

 

4.  Email the application to the Board office at info@nchalb.org with the subject line:  CE Program Approval

5.  The Board office will email (usually within 24-48 hrs) confirmation of receipt and assign a Program ID number to your submission. 

6.  Print the email confirmation with the Program ID and attach a check for $40.00 per program.  Send payment to the Board office.  Include any supplemental documentation which was not submitted electronically. 

DO NOT SUBMIT a hard-copy of the application materials which were submitted electronically.

7.  Applications will be processed in accordance with the guidelines of the current Continuing Education Policy of the Board, which is available on the website:  www.nchalb.org.
